Ketamine infusions
Is anyone using IV Ketamine infusions for acute pain? If so what concentration and rate do you use and what does it work for? Many thanks Harriet |

We currently use a ketamine infusion as an adjunct to PCA morphine in the following indications- patients who are too sedated with opioids but have inadequate analgesia patients with existing, or at risk of neuropathic pain patients with opioid tolerance. Gillian Chumbley, Nurse Consultant at Imperial College Healthcare Trust did a very good talk on Ketamine use in Acute pain at the RCN Pain Forum conference. Might be worthwhile contacting her too. |

Hi, as below we are using ketamine (racemic mix) for the same kind of patients. To avoid confusion we are using the 10mg/ml concentration available in 20ml vials. Drawn up neat into a 50ml syringe and administered through a locked Grasbey 3300 at a rate of 0.5-1.5ml/hr (5-15mg/hr). I hope this is helpful. |

We have just started to use Ketamine in some of our PCAs, along with the morphine or fentanyl. We are in the process of writing some guidelines. If you have any guidelines, would you be interested in sharing them with us?

We have just introduced the guidelines, and at the moment we have not had any patients on the ward requiring it.We have no problem with patients on the ward having s/c infusions as long as the staff are trained. I have used oral ketamine only a couple of times, so i'm afraid I am not the person to ask about that.
