I am currently investigating an individual who was admitted to the QE II Hospital at Welwyn Garden City 7th September this year. He gave false details and was subject to a number of unnecessary procedures as well as receiving a two hourly injection of Pethidine throughout his stay. On 13th September he was arrested on suspicion of fraud after discharging himself from the QEII. He was released on Police Bail pending further enquiries with a view to charging him under section 2 of the Fraud Act 2006, Fraud by False Representation. He is due back at Hatfield Police Station on 6th November 2007.

Modus Operandi

Subject presents himself at the hospital complaining of abdominal pain, or a bowel obstruction. He gives false addresses, normally in North America. Often claims just to have arrived in the UK and that immigration are holding onto his passport and other documents, so he cannot prove his identity.

Insists on injections of Pethidine as pain relief, he is then subjected to numerous medical tests. When it is established that there is nothing wrong with him, he discharges himself, failing to complete any paper work. Several Trusts have tried to bill him as an overseas visitor, without success.

Suspected to be a sufferer of Munchausen Syndrome, although declines any psychiatric examination.

Will engage staff, patients and visitors with strange stories, such as he was a vet for the World Wildlife Fund, working with elephants and gorillas in Africa, or orang-utans in Borneo. He has claimed to be a paramedic in the States, a helicopter pilot in Indonesia, or has served with the army in Asia and had previous operations in Russia and Kazakhstan. Will always provide an excuse as to why he has no notes or medical history available.

Description of Subject
White Male, 5’6”, long darkish grey hair, has a strange accent, American twang, but slips out of it occasionally. Has extensive scarring to abdomen from apparently unnecessary procedures. Currently has an open wound and carries a stoma pouch, although never anything in it. He has a wound which may be infected. On other occasions he has had a Hickman Line, either on his person or in his possession. He carries a large holdall full of clothing and a smaller bag with his personal items, this is where you may find his ID.

As you are using the code "Hawkeye" I assume you have all the information regarding this gentleman collated by Sarah Coulling at St Helliers.

I first came across this man 3-4 years ago in Maidstone. I know he had a recent admission in East Kent where he was challenged and discharged.

Thanks Jane, yes this is our friend. Henry has taken statements and info from us, and colleagues at N&E Herts and the Royal Free who notified the Counterfraud Office. He was arrested and is now on bail. Due to a new fraud law of some kind i believe the Counterfraus office can only act on information from 2007. I have updated the spreadsheet of his profile we collated with everyones help, if anyone wants a copy please email me. A Fraud Notice has also been sent out to all Trusts which you should see. Hawkeye challenges our raison d'etre 'duty to care' regardless of his fake ID's and stories he is unwell. Professionally we have to consider what "appropriate action" we should take to "ensure that procedures and controls are adequate to prevent any adverse events occurring" especially when psych teams say he is competent...I thought 'fictious disorder' (which must be what he has) falls into the category of mental disorder. Does anyone have any comment? he is after all injuring himself and causing us a lot if expense in the mean time. If confronted by psych teams/overseas officers/pain teams with too many questions or he has his opioids withdrawn then he takes flight so treatment is difficult...It will be interesting to see the result of his fraud case, lets hope some sort of medical care will be insisted upon

Sarahs point re Hawkeyes flight when anyone questions to closely is one of the reasons why we encouraged Henry to post here... If we are all aware of this chap we can contact the fraud people who if it is Hawkeye can involve the police etc, and mount a joint comprehensive response without letting him know we know.

The other consideration is that the majority of us in the South East are aware of him, and now the police are involved it may be that he widens his net and travels to other parts of the country......

This patient rings some bells from about 6yrs ago. Unfortunately I cannot remember his name. He said that he was a paramedic. Said that he had just come over from america. Had a Hickmann line. Had scaring from previous laparoscopies. Required large amounts of pethidine. He was about 5'6" but had short hair at the time.
He self dischrged.
Not sure if this will be any help as it was so long ago and may not even be the same guy.

This sounds like a patient who attended Weston in 2004 name of David Norton. History on admission was, lives in British Columbia sustained an abdominal injury whilst working on an oil rig in Saudi.

Sounds like our guy. 2001 is the furthest back we've recorded. Paula, is 'Weston', Weston-Super-Mare? He's due to check in off bail next Tuesday, be interesting to see what happens....a mindful of medico-legal issues.

We're sure that this description fits that of a man admitted to Freeman Hospital in Newcastle a year or so ago. Nobody can remember the name he used (the ward are looking into it now) but we can all remember the story he gave!
He said he lived in America but had been working in the Middle East for a Russian oil company as a paramedic. He had an "accident" on the oil rig and had massive abdominal surgery in Russia, but there was no point in us trying to get these notes because they were all in Russian and the hosital had now closed down.
He had just arrived in this country because he needed to renew his passport (his old passport was at the Americal Embassy) but became ill on the plane and needed to be transferred straight to hospital so had no documents with him.
He had an infection in his abdominal wound and he also had a Hickman line which was to ur amazement inserted upside down - when challenged about this he said that it was inserted in Russia and they had different ways of doing theings there! He was observed "poking" his wounds and dressing on several occasions.
Several efforts were made to establish who he actually was but without success - he went as far as to claim that his duaghter was coming over from America with his medical notes for us.
We refused to give him pethidine but he did receive some doses of morphine and IV antibiotics. He had several non-invasive investigations which were all normal.
He wasn't very happy about his analgesia but stayed in hospital a few days before self-discharging.

Hi Angela,
Thanks to Paintalk, STAPG and RCN network feedback we've traced him back to 2001. Currently he is off radar, the most recent visits being the midlands and Gateshead.
He DNA'd his court hearing on 6/11 and a police warrant for his arrest is expected to be out soon, to be disseminated to Trusts, through Paintalk and Police National Computer.
The CPS are building a case re: fraud and costs to the NHS. We're also concerned at costs to himself re: long term harm
