strenghth of pre addedd fentanyl mix

I wonder what strength of pre added fentanyl /bupivicaine mix hospitals are using for General Surgical patients including colorectal and vascular patients.We currently use the 2 mcg/ml with bupivicaine 0.1% and were looking to change over to the 5 mcg/ml with bupivicaine 0.1%

We use 2mcg Fentanyl with 0.125% Bupiviacaine, for all our general surgical patients and we have found this to be very effective and have had few opioid related side effects.

0.1% & 2mcg/ml (running @ 7-15mls/hr) for us down here in Somerset.

We use 0.125% with fentanyl 2mcg/ml

We use 0.1% levobupivacaine with 2mcgs fentanyl for the majority of patients. On ocassion we use 0.1% bupivacaine with 5mcgs fentanyl. The 5mcgs are used less frequently and usually stepped down to 2mcgs within 48hrs to prevent side effects. (We have had mild auditory and visual hallucinations in patients with 5mcgs at 8-12mls/hr when they approach the 72hour period.

We use 0.125% and 2mcg/ml here in Crewe. We do allow 4mcg/ml but I have not known anyone to use it.

Hi Drena,
We use 4mcgs fentanyl and 0.125%. A couple of our anaesthetists feel this is too strong as a few patients become nauseous or confused on day 2 or 3. Also we feel having 2 different strengths is just asking for trouble!

0.1% and 2mcg/ml here in Swindon (rate 4-12ml - auditing at present to maybe increase max rate)

You'd have to do a systematic search for the latest evidence but i believe you should find, for post-operative epidural analgesia, combinations of bupivacaine 0.1% and fentanyl 4% and adrenaline (epinephrine) are superior. However...they arn't the cheapest! There's a conference next month debating the topic, advertised here (www.STAPG.org)
BTW NPSA alert21 (March 07)recommends Trusts just use one strength to avoid risks.

we use a fentanyl 2mcgs/ml and 0.15% bupivocaine infusion.infusion rate 0-15mls/hr, average rate 8mls,but usually less for elderly patients(4mls/hr).
bolus dose 5mls. we are not yet useing patient controlled epidurals
