Epidural charts
We're just redesigning our epidural chart and at the moment we document BP,P,Resps,02 sats,sedation,N&V & pain scores on the general obs chart and sensory levels, motor loss, epidural site condition and pump observations on another chart, I just wondered how many of you document all the observations on an epidural specific chart.The system we have got at the moment is not working, any help would be much appreciated.

We have a chart which incorporates the general obs and the epidural obs. We found before that if you have separate ones staff forget to complete them, also if boluses are given it is difficult to accurately record the subsequent observations. |

All our obs are done on the EWS charts. Separate charts would not be completed Cx

We use epidural prescriptions which incorporate epidural observations, management of side effects and a section for the Acute Pain Team or on-call anaesthetist to document information on. They work really well, and we have had few problems with them.
