epidural and pca observations
How often are observations done on your patients with PCA and epidural infusions, and is this based on any research findings or just agreed locally. We are trying to combine our PCA and epidural charts with our standard observation chart which includes monitoring the patients mews score however the frequency of observations seems to be causing a lot of disscussion!

Hi Alyson, Our Trust uses a generic 'level one' observation chart for all patients on IVPCA or Epidurals. The timing of observations (agreed locally after research and audit) differ between patients on IVPCA or Epidural. IVPCA Observations are: 15 mins for 1 hour 1 hourly for 8 hours 4 hourly until discontinued Epidural Observations are: 15 minutes for 1 hour 1 hourly for 12 hours 2 hourly for 12 hours 4 hourly until discontinued. At our trust we have seperate epidural and PCA prescription/observational charts, which we have used for some time now and find it less confusing for the ward staff. As well as having information regarding site and depth of the catheter, initial rate and loading dose. They also have information regarding the management of any side effects that may occur, and a section which identifies drugs that can be administered to manage these side effects,and a place where the nurse signs to say they have been given reducing delays in administration. We record our epidural observations 1/2 hourly for the first 4 hours, hourly for 12 hours then 4 hourly thereafter until the epidural is discontinued. We did origionally ask that observations were recorded every 15 minutes for 4 hours but found that they weren't recorded as the ward nurses didn't have time as they often had more than 1 epidural on the ward at the time. Since changing to 1/2 hourly observations in the initial post op period we have found compliance much better. PCA observations are recorded 1/2 hourly for 2 hours hourly for 4 hours and then 4 hourly thereafter. Hope this information helps.
