Acute pain management in the drug abusing patient

Hi
If anyone has any policies/guidelines on this subject I would be extremely grateful if they could forward them to me. I have tried to contact both the previous people who discussed this issue last year but have been unsuccessful

Hi Tracey,

I'm sure this topic was discussed at a Pain Network Meeting at Poole a few years ago and they devised an algorithm using Oramorph for this group of patients. It may be worth contacting them? We work closely with the Substance Abuse Team as we have no idea how much Heroin you get for twenty quid! If I find the algorithm I will send it to you. Hope that helps x

The big issue is not how to treat their pain (as has been mentioned oramorph is good, failing that i.m, with I.V as the 'last-resort')

The real issue is persuading ward docs that the patient should be given opiates (and that the dose is very likely to be higher) in my experience many docs believe that the best treatment method is to with-hold pain relief!

Hi Tracey
I did post a similar question last year.
I am seeing far more patients with drug abuse issues (like most of us I guess)and am trying to get the pharmacy department to help me move a protocol forward on this.
I have attended several study sessions/seminars but still feel I lack knowledge on this.
I turn to the local drug centres for advice for specific patients and I hope shortly to spend some time with them. There seems to be very low knowledge/interest within my Trust on this important issue.
